HOW TO REGISTER

Welcome to College Way Surgery

We are pleased to accept registration requests from patients who are living within the area indicated on the map below:

Please click the following link to see our practice boundary and check your postcode is within our area.

 

Visitors moving to the area

Visitors moving into the area are welcome to register at this surgery.

Please note that we only receive patients who are entitled to receive NHS treatment and whose permanent address lies within our practice catchment area above.

 

Registering as a patient

All new patients are requested to attend in person in order to complete their registration at the practice during our normal opening hours.  To verify identity and residence within our practice catchment area please provide one piece of photographic identification (e.g. passport or photo-card driving licence) and a proof of address (e.g. utility bill).  Each new patient must complete our patient registration form, a copy of this will be provided by a member of the Reception team in the surgery.

Registrations during the COVID-19 pandemic

During this period you are able to download our registration form from the link below.

Please print, complete all sections, sign, and together with a copy of two forms of ID (one photographic) and a picture of yourself (selfie) holding your photographic ID, either post to us or click here to send via email

If you are registering a new baby, please make sure you also include a copy of their birth certificate, or discharge summary from the hospital.

Please be aware that we can only accept registrations to this email address and you will not receive a reply.

**Please note that if you are on regular medication then you will need to make a telephone appointment with the doctor a few days after registering so they can re-add your medication to our system**

 

Child Registration (<16yrs)

Please provide proof of identity and address for each child.  We request sight of each child’s birth certificate and their parent’s identification to match against the child’s birth certificate.  An adult with parental responsibility should be registered at the same practice with the child.   We do not require the child to be present at registration if it is not convenient for them to attend.

 

Armed Forces

When registering for the first time after leaving the Armed Forces, please complete our standard new patient registration form and provide your FMED 133 Document (required to authorise release of your medical records from Defence Medical Services).

 

Temporary Residents.

Please note that requests for repeat medication should be obtained from your registered GP and a prescription sent electronically to a local pharmacy.

If you need to see a GP for an acute condition whilst staying temporarily within our catchment area, please attend the surgery in person to complete our temporary registration form.  Please provide photographic ID and proof of the address within our catchment area that you are staying at (e.g. if staying with a friend or relative please provide a recent utility bill confirming their address).

 

Doctors lists

Our reception staff will inform you which doctors' lists are open and you may express a preference for registering with a particular doctor.

We do our best to accommodate your wishes but please be aware that we cannot register you with a doctor whose list is closed.

We do encourage families to use the same doctor where possible. We feel this to be in your best interest.
