Appointments & Prescriptions Online

To register for this service, please print off and complete the form attached below and bring to the surgery with an acceptable form of photographic identification (e.g. passport, driving license), as well as a utility bill dated in the last 3 months. Once these have been provided we will issue you with a username and password for your online account.

Please note we only offer this service to patients over the age of 15 years.

Online access to your medical records

It is up to your GP to decide if you should be allowed access to your online records. On very rare occasions your GP may think it is not in your best interest for you to look at your medical record online. If this happens, your GP will discuss their reasons with you. Please remember that GP records are important and you should keep them safe and secure, just like your bank account details. No one should force you to share GP records. You have the right to say no. If this happens tell us as soon as you can.

If you wish to apply for access to your medical records online, please attend the surgery in person and bring two forms of ID (one photographic) and ask for a form.
